Neonatal hypocalcemia can have numerous causes and is common in premature infants. We describe a late preterm infant with asymptomatic hypocalcemia whose mother had taken topiramate throughout pregnancy. Other factors that may contribute to hypocalcemia, including being born to a diabetic mother, were excluded in our patient. The patient we report required calcium supplementation for persistent hypocalcemia, most likely due to fetal topiramate exposure.
