Pulmonary vascular resistance (PVR) is increased to some degree in all patients with congenital diaphragmatic hernia (CDH). In patients with severely increased PVR, closure or restriction of a previously patent ductus arteriosus (PDA) can lead to hemodynamic compromise secondary to acute heart failure. Maintaining patency of the ductus arteriosus is not a well-established therapeutic strategy in this patient population. This report examines the potential benefit of maintaining patency of the ductus arteriosus by discussing the pathophysiology and reviewing the published literature.
